Effective Immediately:

ALL paperwork will need to be provided to the International Student Services Office before an I-20 can be issued. Please allow 2-3 days for the review process. If required paperwork is missing NO I-20 will be issued till the required paperwork is provided to us. Upon review of the required documents and I-20 will be issued. If you would like to pick up the I-20, an appointment will be required. No walk-in I-20's will be done.

Application for Admission - (required) This document is required to issue the I-20 with the Diocese of Davenport. This document needs to be filled out completely and accurate, this is used to issue your I-20. Please fill out application completely and send to the Diocese of Davenport.

Declaration of Finances - (required) This document is required to issue the I-20 with the Diocese of Davenport. This document needs to be filled out completely along with a copy of the family bank statement to the Diocese of Davenport. This will be sent along with your I-20 for the consulate to review during your interview process.

English Proficiency Requirements -(required for all grades)

Elementary Students -we have located an English Proficiency Test that is more appropriate for those students coming into our school in elementary K-5. If you are this student please click here and print out a copy of the test, fill it out and mail in with your I-20 request packet. Our office will calculate the answers. Middle School Students - we have located an English Proficiency Test that is more appropriate for those students coming into our school in the middle school grades 6-8. If you are this student please click on the link and print out a copy of the test, fill it out and mail it in with your I-20 request packet. Our office will calculate the answers. English Proficiency Test - Middle

High School students need to complete this document. It is required along with 2 letters from English teachers or tutors, 1 page written report on why you want to attend our school, and the internet evaluation page or a hand circled hard copy document. This gives us an idea of your learning ability. Each school has different requirements for graduation so if you plan to graduate from our school, you will need to consult with a counselor upon registration so they can analyze your grades and give you a projection year for graduation. If you want to attend a elementary school, this test is on a high school level. Please consult with the principal on how they can assess your child. This needs to be sent to the Diocese of Davenport.

Standards of Conduct - (required) - this document is required along with the Application for Admission, Declaration of Finances and English Proficiency test. This gives you a list of what is acceptable and not acceptable to be a student in the Diocese of Davenport schools. Failure to maintain these conducts could result in dismissal from our program.

Intent to Transfer to Another F1 School - (required if wanting to transfer) - this document is required to be filled out completely before a transfer to another F1 school will be granted. This information is required for the transfer process to take place.

Intent to Transfer into the Diocese of Davenport School system - this document is required to be filled out completely before a transfer will be done from the current and projected school. This information is required for the transfer process to take place.

Request for Release of SEVIS data for Seniors - this document is required to be filled out completely when a graduating senior from one of our schools will be continuing their education at a SEVIS approved University or College. This information is required for the transfer process to take place.

Tips for U.S. Visa - this document gives you important information on what you can do to insure your Visa request is granted.

 

Frequently Asked Questions (FAQ'S) - this document is a helpful tool with the most common questions asked by families. Who you should contact? Are there fees? etc. Please print this document for future reference to questions you might have.

 

Important Immigration Reminders - this document shows you how to remain legal in the United States and what will happen if these rules are not followed.

 

Maintaining F1 Status - this documents instructs you on how to maintain your F1 status and what rules you must follow. Failing to do so, could result in deportation.

 

What a Consul looks for - this document gives you information you can use when you meet with the consulate. These are items the consulate looks for during the interview.

 

Pre-Arrival Manual - this document is a helpful tool on what you should bring and not bring to the U.S. Handling your finances, legal status and documents.

 

Visa Denials - this document gives you information on why Visa's are denied, please review and make sure you are compliance with the regulations.
